Webinar series Human Factors
Module 4: Error Culture – From Scapegoat to Shared Learning
Why resilient organisations do not punish errors but learn from them
Errors are part of every organisation.
The decisive question is therefore not whether errors happen, but how organisations deal with them.
Are errors concealed out of fear of the consequences? Are culprits sought instead of causes being understood? Or are errors, near misses and uncertainties used to get better together?
In complex organisations in particular, the way errors are handled determines whether risks are identified early, experiences are shared and lasting improvements become possible.
Human factors influence whether people recognise risks and whether they speak up about errors. Organisations should take them into account and create conditions in which openness, trust and shared learning become a matter of course.
The focus is therefore on questions such as:
- Why do people often not speak up about risks or errors?
- Why does assigning blame prevent lasting learning?
- What role does psychological safety play?
- How do learning organisations come about?
- Which principles do high-reliability organisations use to learn from errors and near misses?
Aim of the webinar
You will learn how organisations can develop a safety and learning culture in which people take responsibility, communicate openly and get better together. A well-developed safety culture ensures that errors are handled appropriately and thereby prevents the same errors from recurring.
You will learn how high-reliability organisations deal with errors and near misses, and which ideas can be derived from this for a sustainable learning culture in companies, public administrations, local authorities, government agencies and other organisations.
